Here is what it actually does. Your muscles use a molecule called phosphocreatine to regenerate energy during short, hard efforts. Heavy sets, sprints, the last two reps you did not think you had. Supplementing creatine tops off those stores, which lets you squeeze out a little more work each session. A rep here, five pounds there. Over months, that extra work compounds into more muscle and more strength. The supplement does not build muscle directly. It lets you train slightly harder, and the training builds the muscle.
